Purpose: To provide resident care and activity needs under the supervision of the Department Manager and/or Nurse on duty. To provide care in a manner that promotes the best possible quality of life for the resident while residing at The Orchard. Minimum Eligibility Requirements Licensed CNA, Home Health Aide, Personal Care Home Maker, or related experienceAbility to read and write EnglishMust be able to communicate effectively with residents, families, staff, community officials, state officials, and general public.Must have compassion for and desire to work with elderlyMust meet all state health requirementsAbility to perform all duties and responsibilities under essential job functions Essential Functions Document resident activity on approved forms in a manner that is legible and understandableProvide resident care including bathing, dressing, grooming, toileting, changing bed linens, positioning, transferring, and incontinence care while using appropriate techniquesObserve any changes in resident condition, take action to ensure safety of resident and report to supervisorMake rounds as directed to check on resident statusRespond to call lights in a timely mannerVital sign monitoring will be performed as needed by personnel with current training to do soAssist with dining service and activity program as directedUnderstand and follow safety and infection control policies and proceduresUnderstand and follow resident care plan and document accordinglyHear, understand, and respond to resident communicationReport incidents and completes appropriate paperworkCollect, wash, and care for laundry & linens in accordance with The Orchard policy.Help to properly maintain all equipment, and general facility maintenance & cleanlinessEnsure work areas are clean and safe, ensure policies covering infection control, health and safety, etc. are strictly followedEnsure compliance and understanding of all regulations and resident rightsAttend all required in-service trainingsPromotes and encourages residents to participate in activitiesAnswers phone appropriatelyMeets customer service standards in a friendly, helpful, courteous manner with resident satisfaction a priorityAbility to distinguish and prioritize duties resident needs (both emotion & physical) v/s task at handThe job description provides a framework for the job, other duties may be assigned as necessary Duties (this is not an all-inclusive list): Clock In / Out correctly Receive report / Give ReportServe Meals/Feed Deliver Meal Trays / Pick Up Trays Change Bed LinensAssist Ambulatory residents w/ toiletingChange DiapersBathe, Dress, Groom ResidentsAssist w/ Oral HygieneBrush HairNail Care as directedRespond to call lightsReport unusual behaviorPlace soiled linen in appropriate containersRemind/Escort Residents to ActivitiesEscort Residents to MealsKeep resident rooms clean and uncluttered Walk ambulatory residents Insure residents have fresh waterEncourage fluid intake as directedDocument Bowel/Bladder activityDocument IntakeInsure residents are clean, dry, and comfortable before leavingSign/Date/Time all documentationFollow Infection Control ProceduresUse good body mechanicsPartial Bath / Sponge BathRange of Motion ExercisesBed ShampooShaving / Oral HygieneMake BedTransfer ResidentsUse gait beltMeasure WeightInsure Resident SafetyCare post deathDementia Care Working Conditions Ability to work schedule and communicate availability according to policy Physical/Sensory Requirements (with or without the aid of mechanical devices) Medium Work Ability to exert 20-50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to lift, carry, push, pull, or otherwise move objects.
